Due to COVID-19, for the health and safety of all our Village community, we want to ensure that receiving and providing services is as safe as possible for both Members and Volunteers. To that end, we are asking that Members and Volunteers who would like to be eligible to receive and/or provide BVHH services provide us with the following:
- Proof of complete vaccination and booster, which can be sent to us via email or US mail. If you don’t have access to a means of copying and sending your vaccination card, please call (415 508-2185) or email ([email protected]) and one of our board members will be happy to come by your home and take a photo of it for our records.
- A signed and dated Brisbane Village Helping Hands Communicable Disease Waiver and Release. This form can be returned to us via email or US mail. If you don’t have a means of printing out the Waiver, please call or email (see above) and we’ll provide you with a hard copy.
